Iowa's Medicaid line for babies under one is 305% of the poverty guideline with the disregard, $6,943 a month for a family of three with a newborn, while pregnant women now qualify up to 220%, $3,967 a month counted as two, after the state's own income guidelines lowered the pregnancy line in April 2025. Older children are covered by Medicaid and then Hawki, the state CHIP, up to 307%. Iowa also expanded Medicaid for adults, to $1,835 a month for one person. SNAP, which the Department of Health and Human Services calls Food Assistance, tests gross income at 160% of poverty, $4,400 a month for four in fiscal year 2027, with no asset limit. A Des Moines family of four earning $3,700 with $1,050 rent and $300 of child care would receive about $410 a month, an amount the federal $3,575 line would have ruled out.

Pregnancy at 220%, babies at 305%

The CMS table of December 2023 showed Iowa with the highest pregnancy line of any state. Iowa's Department of Health and Human Services now publishes lower figures: 215% of poverty for pregnant women and 300% for infants, before the 5% disregard, in force from April 1, 2025 (Iowa HHS income guidelines). A nurse in Cedar Rapids who earns $6,000 a month and is expecting her first baby is at 333% of the guideline for two and is above the pregnancy line, so she would buy a Marketplace plan with the premium tax credit. Once born, the baby has a higher line than the mother had, 305% until the first birthday. After that, Medicaid covers children to 172%, and Hawki, for ages 1 up to 19, continues to 307%, $6,989 a month for a family of three. The family's income is measured each time with MAGI rules.

Adults outside pregnancy are covered under the expansion to 138%; Iowa's old parent standard, a dollar amount CMS converts to 44%, no longer sets the limit for parents. Above Medicaid, Iowans use HealthCare.gov and the premium tax credit.

Food Assistance at 160% of poverty

The USDA chart lists only Iowa at 160% (USDA BBCE chart), between the federal 130% and the 165% used by Illinois. For four people, the Iowa line is $4,400, against $3,575 under federal rules. The Des Moines family's $3,700 of wages falls between the two. Its deductions: $740 for earnings, $229 standard, $300 of child care, leaving $2,431. Rent plus the $554 utility allowance is $1,604, and $389 of it is deducted. Net income is $2,043 and the benefit $410 out of $1,023. Categorical eligibility spares the family the $2,750 net test. Without the child care bill on file the estimate would be $275: each dollar of care is worth about 45 cents of benefit here, because lowering income also enlarges the shelter deduction. Ames and Iowa City lead the HUD table

Story County, home to Ames, has Iowa's highest very low income limit for four, $61,200, just ahead of Johnson County, around Iowa City, at $60,700. Polk County and the Des Moines metro share $58,200; Linn County, around Cedar Rapids, is at $52,650. Iowa covers adults up to 138% of the poverty guideline under the ACA expansion. Children's coverage above Medicaid is called Hawki. Since April 1, 2025 Iowa Medicaid covers pregnant women up to 215% of FPL (down from 375%) with 12 months of postpartum coverage, and infants under 1 up to 300% (the April 2026 table; a 2025 fact sheet says 302%); children 1-18 stay at 167%.
